Client Intake Questionnaire

Over the years of working with clients, we have prepared the following list of questions, which help guide us through a design project. This list is not meant to be totally complete, but instead functions best as a starting point for helping us understand you, your business, and your customers.

There is no need to complete all questions if they are not relevant to your project. However, being as complete as possible will help us meet your goals for your design/development project.
General Questions

1. What functionality does your site need (please be complete)? For example: blog (can be used for article/news posting), shopping cart, contact form, Facebook integration (and how you would like it to integrate), Twitter integration (and how you would like it to integrate), other social/sharing features, any other functionality (product ratings, product reviews, wishlist, etc.).
2. As completely as possible, can you list the pages your site will need (e.g., "Home", "About", "Products/Shop", etc.)? Are you removing any pages from your current site, or adding any new ones (if applicable)?
3. Please list 5-10 "example" sites that you like. We are not looking for sites to copy, instead could you please list what you like (or don't like) about each, and how you would like your site to be similar. In your opinion, what do these sites accomplish, and how would you like to accomplish the same thing(s)? Please tell us what you like about each site IN DETAIL.
4. What overall tone are you going for? That is, what five words would you use (and like your customers to use) to describe it? Examples could be clean, edgy, vibrant, simple, powerful, etc.
5. What color combinations do you like? What color combinations do you really not like?

About Your Project
11. What are your overall goals (specifically, business objectives) for the site? That is, if you could fast-forward yourself a year, what sorts of results from this project would make you happy?
12. What is the primary call to action (e.g. fill out a contact form, sign up for our email list, etc.) on the website now? That is, what the the most common action you would like people to take after coming to your site? Do you see this being the same going forward? How aggressively would you like to push various calls to action?
13. What are the highest trafficked pages on your current site? Or, what pages are people spending the most time on, and/or are most important to your customers’ online experience?
14. What do you want the site to not be? That is, can you give some examples of sites that aren't doing what you want to do as well as you want to?
15. Who is going to be maintaining the site? Additionally, who is going to provide all of the photography and content for the site?
16. What are the things your customers/visitors are most likely to do on their first visit to your website? Do these functions change on subsequent visits? What would you like every first-time visitor to your website to know about you?
17. What are the most important uses a mobile user would have of your website? That is, if you imagine the mobile (e.g. smartphone) view of your website only having 3-5 buttons on it, what would those buttons say?
18. In your opinion, what is the major missed opportunity on your current website (if applicable)? What is the one thing that makes you think, "if our website only had _______, we would get more phone calls"?
19. When speaking with prospective clients, do they ever say, "Wow, I didn't see that on your website"?
20. Is there anything you would like to add about your project, anything we missed? Is there something else that your project should include in order to be maximally successful?
